BENEFITS
NVFS offers a variety of benefits to staff who work a minimum of 51% time or more than 19.125 hours per week. These benefits include health, dental, vision, life insurance, a flexible spending plan, income protection, a retirement savings plan, annual and sick leave. Detailed information is available through specific plan documents, copies of which are located in the Human Resources Department. The agency shares a significant portion of the rates for health, dental and vision insurance for employees working 100%. NVFS' contribution is prorated based on the percentage of time worked for employees working less than 100%.


HEALTH INSURANCE
NVFS offers three Kaiser Permanente health plan options to suit the needs of employees: the HMO Signature Plan, the HMO Select Plan and the POS Plan.

DENTAL INSURANCE
NVFS offers AETNA dental insurance. This is a dual option DMO/PPO that provides 100% coverage for preventive care, such as cleaning, basic, simple tooth extraction and major periodontal and endodontic services.

VISION INSURANCE
NVFS offers VSP Vision Insurance which provides for eye exams and lenses every 12 months and frames every 24 months, each with a $10 co-payment.

LIFE INSURANCE
NVFS provides group life insurance equivalent to one time your annual salary up to a maximum of $50,000 for benefit eligible employees after completing four months of consecutive employment. In addition, AD&D coverage is provided for benefit eligible employees (after completing four months of consecutive employment) in the event of an accidental bodily injury, which causes loss of life or limb.

LONG-TERM DISABILITY
NVFS provides long-term disability insurance for benefit eligible employees after completing four months of consecutive employment. Premiums are paid by NVFS. The benefit pays 60% of your monthly base salary to a maximum monthly benefit of $5,000 and is payable after a 90 day elimination period.

VOLUNTARY LIFE INSURANCE
Benefit eligible employees may purchase additional term life insurance up to a maximum of $500,000. In addition, AD&D coverage is also available for benefit eligible employees in the event of an accidental bodily injury, which causes loss of life or limb. Voluntary Life Insurance is also available for a spouse and/or children of benefit eligible employees.

FLEXIBLE SPENDING PLAN
This plan enables NVFS benefit eligible staff to have a specified amount of pre-taxed money deducted from your paycheck each pay period. These dollars are set aside in a reimbursement account and subtracted from your gross earnings before any taxes are taken out.

ANNUAL LEAVE
NVFS benefit eligible employees receive Annual Leave based on length of service, and if less than full- time, prorated based on percentage of hours scheduled to work.

SICK LEAVE
NVFS benefit eligible employees will receive 15 sick days per year prorated based on percentage of hours scheduled to work. Accrual is capped at 675 hours.

PAID HOLIDAYS
All benefit eligible employees are entitled to ten holidays each calendar year--eight Federal holidays, one floating holiday, and the day after Thanksgiving.

401(K) RETIREMENT PLAN
All NVFS employees over the age of 21, (except those who are non-resident aliens with no U.S. source of income and those who are treated as independent contractors), may opt to have a percent of their compensation up to the maximum amount permitted by law deposited in a 401(k) plan. After one year of employment, NVFS will contribute six percent (6%) of the employee’s salary into the 401(k) plan for employees working at least fifty-one percent (51%). The minimum vesting period to receive a portion of NVFS contribution to the 401(k) plan is three years with full vesting after five years. The 401K-contribution limit for 2007 is $15,500; for anyone over age 50, an additional $5,000 can be contributed as “catch-up” contribution.

NVFS offers affordable, convenient legal services which can be utilized through the Hyatt Premier Legal Plan and is available to NVFS benefit eligible employees.
