HEALTH CARE: FAIRFAX COUNTY

Program: Fairfax Medical Care for Children Partnership (MCCP)
Provides low-income, uninsured children with access to health care services through a network of private-practice medical care providers. Serves families with incomes up to 250% of the federal poverty level. Services include primary preventive and sick care, limited specialty care, dental treatment, lab services, pharmaceutical assistance, case management and information and referral services.

Program: Adult Health Partnership (AHP)
Provides low-income, uninsured adults with urgent health care needs access to a physician or dentist who will treat the urgent/emergent problem. Serves adults with incomes of up to 250% of the federal poverty level. Services available include limited medical and dental care, pharmaceutical assistance, short-term case management and information and referral services.

Program: Fairfax Accessible Medication Program
A service of the Adult Health Partnership, the Fairfax Accessible Medication Program provides low-income, uninsured adults taking medications for chronic illnesses with assistance in applying to pharmaceutical companies’ patient assistance programs. Serves adults with incomes below 200% of the federal poverty level. Through participation in a patient assistance program, individuals can obtain their chronic medications either free or at a greatly reduced cost from the drug manufacturer.
