COVID-19 EMERGENCY ASSISTANCE PROGRAM

Please read through the resource and information guides from local and national vendors who have indicated their willingness to work with those impacted by the COVID-19 crisis.

There are several categories of impact directly related to the COVID-19 emergency, including:

• Those who have lost or are receiving reduced income from employment as a result of the COVID-19 emergency
• Unforeseen expenses due to COVID-19 emergency, such as healthcare co-pays, childcare, etc.

We are asking all who contact us to utilize this information and other reasonable options such as contacting your lender, landlord, or utility prior to requesting last resort funding. There is a limited amount of funding available and we want to ensure it is considered as funding of last resort. Funds will be distributed on a first-come first-serve basis for immediate needs, according to availability and based on our funders’ guidelines.

ELIGIBILITY REQUIREMENTS

In order to qualify for assistance from the COVID-19 Emergency Assistance Program, the recipient must satisfy all of the following requirements:

1. Be a resident of Alexandria, Arlington, Fairfax County, Fairfax/Falls Church, Loudoun County, Manassas, Manassas Park, or Prince William County
2. Have been directly impacted by the COVID-19 emergency creating financial hardship (must show documentation as indicated below)
3. Provide tangible documentation of the COVID-19 related hardship

The process

Our goal is to complete this process for you as quickly as possible.

1. Fill out the application form at the bottom of the page
   • For assistance completing the form and assembling supporting documents, please call 571-748-2552. This number is only for help in submitting the application.
2. Assemble supporting documents as appropriate and described in the next section
3. Provide supporting documentation in one of three ways:
   • Upload documents via the form below
   • Email supporting documents to covid19@nvfs.org
     • Please write the name of the person requesting assistance on the top of all documents
     • Please indicate in the body of the email the name and address of the person requesting assistance
   • Print and fill out the application form and fax to 571-730-2860
     • Fax cover sheet must indicate the applicants name and address
     • Please write the name of the person requesting assistance on the top of all documents
4. You will receive an email verification of receipt of your application within one business day. Please do not call or email regarding the status of your submitted application.

Some examples of documentation that will be required to complete assistance:

• Identification: Copy of photo ID containing residential address
• Documentation of Loss, for example:
  • Copy of pre-emergency and post- emergency paystubs demonstrating loss of income
  • Copy of reduction in hours statement from employer indicating cause related for those not receiving any pay
• Confirmation of unemployment application
• If requesting assistance for a medical cost or co-pay, please provide a copy of the bill
• For childcare, an invoice or bill indicating the cost
• Other need: If other need is identified please provide documentation of need for consideration

All data is kept confidential and secure.

Based on this information from a complete application, NVFS will review and provide notification of available funding. NVFS will consider all other resources available in the community to meet the need presented (please note: some resources may have additional documentation requirements).

Funds will not be paid to the individual requesting assistance. They must be paid to a vendor, landlord, mortgage or utility company, etc. Checks will be mailed directly to the vendor via USPS.

Prior to fund disbursement, self-attestation of need form must be signed by beneficiary. This process may be completed electronically utilizing email or e-signing.
